① 配布するリストを選びます Select the list to distribute
「基本」「治療を決めるとき」「シニア向け」から、対象や場面に合うものを選定します。
Choose the most suitable option depending on the patient and situation: Basic, When Deciding on Treatment, or For Seniors.
② 配布のタイミングを決めます Decide on the timing of distribution
初回治療説明前・治療選択時・再発告知時など。病棟・外来での標準のタイミングを決めておくと安定します。
For example: before the first treatment explanation, at the time of treatment selection, or when informing about recurrence. Establishing standard timing in wards and outpatient clinics ensures consistency.
③ 役割分担を決めます Define role allocation
配布する人（例：看護師/医療ソーシャルワーカー）と、回答する人（医師）を明確にします。
Clarify responsibilities: distribution (e.g., nurse/medical social worker) and answering (physician).
④ 診察での使い方を揃えます Standardize its use in consultations
診察冒頭でQPLを確認し、優先度の高い質問から回答。未回答は次回までの課題として記録します。
Review the QPL at the start of the consultation, answer questions in order of priority, and record any unanswered ones as tasks for the next visit.
⑤ 患者・家族と共有します Share with patients and families
診療後に配布したQPLへメモを追記し、家族や地域医療とも共有できるよう促します。
Encourage patients to add notes to the QPL distributed after the consultation and to share it with their family members and community healthcare providers.

導入フロー Implementation Flow
単一診療科での導入 For a Single Department
- 標準タイミングを決定（外来初診／治療説明前）
Decide on the standard timing(for example, at the first outpatient visit or before treatment explanation). - 配布担当（看護師/受付/MSW）と回収・共有担当（外来看護師）を決める
Assign staff roles for distribution (nurses, receptionists, or medical social workers) and for collection and sharing (outpatient nurses). - 診察での使い方（冒頭でQPL確認→優先質問から回答）を共有
Align how to use the QPL during consultations — review it at the beginning and answer questions in order of priority. - 未回答項目は次回へ引き継ぎ、カルテテンプレートで記録
Carry over unanswered items to the next visit and record them in the chart template.
複数診療科での導入 For Multiple Departments
- 地域連携室／がん相談支援センターと連携し、院内共通の配布ポイントを設置
Work in collaboration with the Regional Liaison Office or Cancer Consultation and Support Center to set up a shared distribution point within the hospital. - 診療科間でQPLの受け渡し（スキャン/共有フォルダ）を決める
Decide how to transfer QPLs between departments (for example, by scanning or using a shared folder). - カンファレンスでQPLの未解消質問を共有し、説明の抜け漏れを解消

Q. いつ配るといいですか？ When is the best time to distribute the QPL?
A. 様々なタイミングでお渡しいただけます。詳しくは「配布のタイミング」をご覧ください。
It can be distributed at various points during care. For details, please refer to the Recommended Timing for Distribution section.
Q. 外来でも病棟でも使えますか？ Can it be used in both outpatient and inpatient settings?
A. はい。診察室や通院治療センター、相談支援センターや病室など、患者さんと治療について話し合うタイミングであればいつでもお渡しいただけます。受付や待合室で自由にお持ちいただける形にすることも可能です。
Yes. The QPL can be used anytime you discuss treatment with patients — in consultation rooms, outpatient treatment centers, support centers, or hospital wards. You can also make it freely available at the reception desk or waiting area.

A. シニア向けリストを配布し、体力・生活の希望・サポート体制を一緒に確認します。ご家族同席での利用も有効です。
Distribute the Senior Question List and review physical condition, lifestyle preferences, and support systems together. It is also effective to use it when family members are present.
医療者の業務負担について About Workload for Healthcare Professionals
Q. 医療者側の負担になりませんか？ Does using the QPL increase the workload for healthcare providers?
A. 大きな負担にはなりません。患者さんが聞きたいことを事前に整理することで診察時のやりとりが円滑になります。実際に活用している医療機関からは「患者さんの理解度や納得感が高まった」「必要な話題に集中できた」との声が寄せられています。
Not significantly. Because patients can organize their questions in advance, consultations proceed more smoothly. Facilities already using the QPL report improved patient understanding and satisfaction, as well as more focused discussions.
Q. QPLは診療時間を延ばしませんか？ Does the QPL make consultations longer?
A. 事前に質問が整理されるため、説明の順序決定や資料提示がスムーズになり、結果として効率化につながる場面が多く報告されています。
On the contrary, it often helps streamline the process. Since questions are organized beforehand, explanations and materials can be presented in a smoother, more efficient order.
Q. 配布の説明は誰が担当すべきですか？ Who should be responsible for distributing the QPL?
A. 配布は外来看護師・MSWなど非医師職でも可能です。待合室などに置いておき、自由にお持ちいただける形も良いでしょう。診察では医師が優先質問から回答し、未回答は次回に持ち越します。
Distribution can be handled by non-physician staff such as outpatient nurses or medical social workers. You may also place it in the waiting area for patients to take freely. During consultations, physicians answer the prioritized questions, and any remaining ones are carried over to the next visit.

Q. 質問促進リストを使ってもいいですか？ Can we freely use the Question Prompt List?
A. はい。CC BY-NC（表示 - 非営利 4.0 国際）ライセンスに基づき、原著作者を明示いただければ、複製・再配布・改変が可能です。
ただし、営利目的での利用はできません。改変・再配布の際には、改変した旨を明記し、同じライセンス条件（CC BY-NC 4.0）を継承してください。

A. 調査・研究目的で紹介する際は出典（プロジェクト名・URL）を明記してください。リンク共有も歓迎します。ただし、転載や加工を行う場合は事前にご連絡ください。
Yes. When using it for research or survey purposes, please cite the source (project name and URL). Sharing links is also welcome. However, if you plan to reproduce or modify the material, please contact us in advance.
Q. 臨床試験・商用利用について What about use in clinical trials or for commercial purposes?
A. 事前のご連絡が必要です。臨床試験や製品・サービスへの組み込みなどをご希望の場合は、お問合せフォームよりご相談ください。ご利用条件やクレジット表記について個別にご案内いたします。

患者さんへの説明について About Explaining the QPL to Patients
Q. 患者さんにどう説明して渡せばよいですか？ How should I explain the QPL to patients?
A. 「医療者に聞きたいことを考えるきっかけになる資料です」とお伝えください。簡単な説明で安心して受け取っていただけます。患者さん向け説明動画（YouTube）をご案内するのも有効です。
Simply tell them, “This is a tool to help you think about what you would like to ask your doctor.” A short explanation is usually enough to make patients feel comfortable. You can also guide them to the patient instruction video (YouTube link).
